The 18 - 24 transitions team aims to understand young people’s lives from their point of view in order to enhance their potential for change. The team offers mental health, psychology and substance misuse assessments and then uses the results to develop plans with the young people for intervention or onward referral. They work to reduce gang related offending and serious violence and harm to young people, their families and local communities.
